2008-2009 Pre-Kindergarten Registration
May 12-16, 9-11am and 1-3pm
All Pre-Kindergarten students in the district will attend Lake Dallas Primary School.
Our Pre-Kindergarten will be continued at Lake Dallas Primary if a minimum of fifteen eligible children enroll.
A child is eligible for Pre-Kindergarten if the child:
· is 4 years old on or before September 1, 2008, and must be eligible by at least one of the following reasons,
· is unable to speak and comprehend the English language; (an assessment will be given)
· is educationally disadvantaged, (family whose income would meet the same requirements as the national free and reduced price lunch program).
· is a homeless child;
· is the child of an active duty member of the armed forces, including state military forces or reserve component of the armed forces, who is ordered to active duty;
· is the child of a member of the armed forces, including state military forces or a reserve component of the armed forces, who was injured or killed while serving on active duty; or
· is or ever has been in the conservatorship of the Department of Family and Protective Services following an adversary hearing held as provided by Section 262.201, Family Code.
For Pre-Kindergarten enrollment, you will need to bring the child’s:
· birth certificate (original);
· complete, up to date, immunization record that is verified by a physician or health clinic;
· household’s proof of income (if needed for eligibility) ex: 2007 income tax form or payroll stubs for 6 months;
· proof of residency, ex: utility bill, drivers license;
· Social Security card (original); or any
· official documents providing proof for any other eligible reason.
Immunization requirements for school enrollment are as follows:
· Five doses of DPT/TDaP Vaccine (four doses, if one is after 4th birthday)
· Three doses of Polio Vaccine (three doses, if one is after 4th birthday)
· Two doses of Measles/Mumps/Rubella Vaccine (one dose on or after 1st birthday and second dose by age 5 or by start of Kindergarten)
· Three doses of Hepatitis B Vaccine
· One dose Hib Vaccine (on/after 15 months old) (for 4 year old pre-kindergarten program only)
· One dose of Varicella Vaccine (on/after 1st birthday)
· Two doses of of Hepatitis A for children attending pre-kindergarten or early childhood.
· One dose of pnuemococcal vaccine for children ages 24-54 months attending pre-kindergarten or early childhood
