A new Food Allergy Action Plan is needed each year. Please let the school nurse know if your student will be carrying an epinephrine auto injector or Benadryl with them. An Emergency Medication Self-Carry Agreement will need to be on file.
A new Asthma Action Plan must be on file each school year. Please make sure your student has any inhalers or nebulizer medications needed while they are at school.
