High school offering $25 physicals for students on May 3
Lake Dallas High School will offer $25 physicals for next year’s freshman to senior students on Monday, May 3, providing families with an inexpensive opportunity to receive the necessary documents instead of paying for a personal physician visit.
Physical forms will be passed out to each student athlete on Monday, April 19, during the athletic period. Each physical will also include BMI measurements and an ECG. Students must return their completed forms to their coaches by Friday, April 30. This includes all “yes” answers explained and parent and student signatures. Forms should be accompanied by $25 in cash or check (payable to the LDISD Trainers Activity Fund).
Administration of physicals will be scheduled by teams as follows to ensure a speedy process:
5 p.m. Football and Volleyball
5:45 p.m. Basketball (Boys and Girls)
6:05 p.m. Cross Country, Golf and Tennis
6:20 p.m. Soccer (Boys and Girls)
6:40 p.m. Baseball and Softball
7 p.m. Cheer and Dance
7:20 p.m. Band
Students can schedule a time during their athletic period, prior to the day of physicals, to complete the BMI, blood pressure and vision check portions so the physical administration will proceed faster on May 3. Students or their parents can contact Coach Debby Winder or Coach Marissa Parker, athletic trainers, to schedule these early; students must bring their completed forms in order to complete these portions.
